The Minyan is pleased to be planning the 5779 edition of its Memorial Book. We invite members and friends to include the names of loved ones for whom they say Yizkor, to be printed in a booklet that will be available in shul on days that Yizkor is said. To submit your loved one(s) name(s) for inclusion in the 5779 edition, please complete the form below and submit your payment.

Donations for dedications are as follows:
First name: $18.
Names 2 through 9: $10 each.
The maximum charge for up to 50 names is $100.

Please note: Our intention is to (re-)print the 5779 Memorial Book to be available in shul on Yom Kippur, Shemini Atzeret, Pesach, and Shavuot during 5779. Names submitted by September 12, 2018 will IY"H be included in the books available on those four Yizkor occasions. Names submitted after that date may be included in an addendum for remaining Yizkor occasions for the year 5779. This donation does not cover dedications for any subsequent years. Bottom of Form
